来自瑞士科研人员的一篇新的综述论文对致力于2030年终结AIDS流行的WHO全球行动的工作进展以及在HIV治疗上所面临的挑战进行了审视。 中低收入群体中接受抗逆病毒治疗(ART)的人数在2006年仅约130万，上述个体在疾病诊断方式、服用的抗HIV药物种类以及治疗方式上均存在相当的差异。为规范和快速实施HIV的治疗，包括简化ART的实施方案，世界卫生组织（WHO）出台了一项公共健康举措，旨在资源贫乏群体中提升ART的实施，并在2030年实现终结艾滋病传播以消除此公共健康威胁的目标。 WHO的这一理念及全球行动的确为提升HIV的治疗奠定了基础。然而，在HIV控制及健康服务全覆盖方面，人类依然面临着诸多挑战。鉴于这些挑战的存在，包括HIV诊断检测技术的提升、HIV治疗的简化以及监测的加强，拯待将HIV检测及治疗和HIV预防服务有机关联。 WHO实施的这一举措要求公共健康核心原则在一些国家得到落实，使得这些国家更加接近于达到控制HIV传播的全球目标。近期由瑞士科研人员发表在The Lancet Infectious Diseases期刊上的一篇综述论文对这项全球计划项目的执行情况及最新进展进行了审视，并就HIV治疗及服务以实现预期目标应采取的进一步举措展开了探讨。 HIV治疗领域的进展 据报道，截止2016年底有超过1950万人正在接受ART治疗。正是由于HIV治疗有效性的提升，使得那些HIV携带者个体的预期寿命接近人群中的平均水平。迄今已经取得了诸多关键研究突破以及诊断技术的创新，同时，从观察性与试验性研究中获得的高质量数据促使FDA批准将16种抗逆病毒新药以及不同复方药物列为HIV治疗药物。此外，一线治疗费用已大大减低：从每位患者每年花费超过1万美元减至不到100美元。 初级医疗中的HIV治疗 与此同时，认同并执行将服务层级从专科医学中心扩展至初级医疗体系这一举措在成人及孩童HIV携带者的预后改善上也起到关键作用。尽管最初在工作重心下移以及将ART整合到初级医疗体系中遇到阻力，但临床效果得以提升，特别表现在高风险群体中的相关证据不断增长，这对WHO的上述举措起到支撑作用，而这一举措正在世界范围内许多国家中实施。 实施WHO在2006年制定的工作指南促进了过去十年间ART治疗的开展，使HIV感染所致的死亡人数减少约780万，同时还减少新增HIV感染病例约3千万，这一效果在中低收入群体即高风险人群中尤其突出，这一举措显著改变了HIV流行的轨迹。为终结AIDS的流行，接下来应将工作重心转移至如何加强HIV的预防即长期的病毒防控及抑制病毒的传播。 何为90-90-90目标？ 为快速跟踪这一行动计划，WHO设定了一个雄心勃勃的90-90-90工作目标：至2020年，90%的HIV携带者知晓其病毒感染状态，90%明确诊断的HIV阳性患者接受抗HIV治疗，相应使90%的接受持续ART治疗的患者其病毒处于抑制状态。实现这一目标将在未来15年内挽救超过2100万人的生命，同时减少新增感染病例2800万。 治疗的简化与标准化 就这一举措而言,重要的是对治疗方案、临床决策以及监测进行简化与标准化处理。每日服用一粒药丸对大多数HIV感染患者而言是一线ART治疗的金标准，因而在使用新药和新方案时必须考虑到这一标准。有关治疗失败方面的管理也拯待提高，一旦一线治疗失败，而需要接受二线治疗的患者可能面临持续不断的挑战时，通常无每日一次的药丸或固定复合药可用，这就需要简化二线治疗。 HIV预防在最近几年中取得了长足的进步，这应归功于包括ART的早期启动、男性包皮环切术和预防接种的提升或阻断感染传播以减少传染的举措在内的各种有效应对策略的综合运用。然而，实施与持续执行预防策略的状况仍不理想，如要实现2030年终结AIDS传播的预定目标，必须加快和强化有关预防干预计划的实施，特别是通过将HIV检测、治疗和服务有机结合在一起，因为仅抓单独任何一个环节均不可能奏效。提高HIV检测的覆盖面可有效保障个体感染病毒后能在第一时间被检出，进而启动处理程序并给予充分的医疗护理，因此上述举措可开创控制HIV感染率的新局面。 综上，自2006年引入WHO理念以来，有关改善HIV治疗及HIV携带者临床预后的公共健康举措已实施多年，然而，这篇综述论文所提供的证据表明，离实现2030年终结AIDS流行的既定目标仍有相当长的距离。因此，上述诸多举措依然具有重要意义，强调需要世界各国尤其是那些HIV感染高风险国家执行有关ART实施和医护方面的关键公共健康方案，旨在使这些国家逐步达到控制HIV流行，并最终实现健康服务全覆盖的目标。 作者：Lacey Hizartzidis, PhD 译者：Mingfa Li,
