Introduction to MIPS
The Merit-based Incentive Payment System (MIPS) was introduced by the Quality Payment Program (QPP) to provide compensation for Medicare Part B healthcare providers based on their performance in value-based care.
MIPS Categories
MIPS encompasses four key categories for reporting, which include:
1. Quality
2. Improvement Activities
3. Cost
4. Promoting Interoperability
Each category contributes a specific percentage to the overall MIPS score, which ultimately affects the payment adjustments clinicians will receive for the following year. It is important to note that the rules and regulations for each category are subject to annual updates.

MIPS 2022 Quality Measures
The following are quality measures specifically relevant to the Certified Nurse Midwife specialty for the MIPS 2022 evaluation:
Measure No. 047: Advance Care Plan
This high-priority measure targets patients aged 65 and older. It focuses on whether patients have an advance care plan, have designated a decision-maker in their medical records, or have documentation of discussions regarding advance care planning. Different protocols apply when patients choose not to appoint a decision-maker, are unable to provide a plan, or designate someone else for care.
Measure No. 110: Preventive Care and Early Detection: Immunization Against Influenza
This measure applies to patients aged six months or older who received an influenza immunization at a clinic or have proof of prior immunization between October 1 and March 31.
Measure No. 130: Documentation of Current Medications
This high-priority measure is for patients aged 18 or older taking multiple medications. Physicians must document all medications in the patient’s medical record during their visit.
Measure No. 226: Tobacco Use Screening and Intervention
This measure is for patients aged 18 and older who have been screened for tobacco use once or twice in the past year. If patients are identified as regular smokers, they should receive interventions to assist with cessation.
Measure No. 335: Elective Delivery after 39 Weeks of Gestation
This high-priority measure pertains to patients who have given birth within the last year, regardless of age. It focuses on those who delivered a live singleton after a gestation period of 39 weeks via elective labor induction or cesarean section.
Measure No. 336: Postpartum Care and Management
This high-priority measure applies to patients who have given birth in the past 12 months. It assesses whether they received postpartum care that includes education on breastfeeding, postpartum depression screening, counseling on family planning, and lifestyle advice.
Measure No. 431: Alcohol Use Screening and Counseling
This measure addresses patients aged 18 and older who have exhibited high-risk alcohol use and received consistent check-ups in the past year, along with counseling to manage their alcohol use safely.
Measure No. 475: Early Detection of HIV
This measure includes patients aged 15 to 65 who underwent HIV testing at the beginning of the measurement period.
