The landscape of hypertension in India has experienced a significant transformation, evolving from primarily affecting the elderly to becoming a widespread issue among younger populations. Recent findings from the Great India Blood Pressure Survey reveal an alarming hypertension prevalence of 22.4% among young adults aged 20 to 44.
Implications of Early Onset Hypertension
This trend results in what can be termed a “silent 20-year head start” on vascular aging when compared to Western countries. Current risk assessment tools, like the 10-year ASCVD risk calculator, often emphasize age as a critical factor, which obscures the extensive “lifetime cumulative exposure” to hypertension that young Indians are experiencing.
Challenges in Detection and Management
A lack of awareness regarding screening and limited access to healthcare contribute to the unnoticed progression of hypertension. This scenario leads to prolonged exposure to elevated blood pressure levels and subsequent hypertension-mediated organ damage (HMOD).
Prevalence of Hypertension-Related Conditions
Data specific to India indicate that left ventricular hypertrophy (LVH) is present in 23.4% of young hypertensives, while microalbuminuria affects 9%, and there is an increase in carotid intima-media thickness (CIMT) among this demographic.
