Increasing Demand and Sales
The global demand for online pharmacies, also known as e-pharmacies, has surged significantly from 2014 to 2023, with sales reaching an impressive $98 billion. Although online pharmacies have been present for years, their accessibility has expanded notably since the onset of the COVID-19 pandemic. This surge in access, however, is accompanied by heightened risks that necessitate robust regulations for these platforms.
Risks Associated with Online Pharmacies
Tainted Drugs
Policing the internet presents a formidable challenge. In March and April of 2022, the U.S. Food and Drug Administration (FDA) issued warnings to various e-pharmacies. Following multiple risk assessments, the FDA identified non-compliance with good manufacturing practices (GMPs). Warnings were directed to both the generic drug manufacturers and the e-pharmacies distributing their products. These assessments revealed risks of cross-contamination and the presence of tainted drugs and impurities. Ensuring safety and standardized operational procedures in handling active pharmaceutical ingredients (APIs) is crucial. While it is difficult to eliminate all forms of impurities, reputable pharmaceutical companies take proactive measures to reduce impurity risks by adhering to stringent drug processing guidelines. The challenge of monitoring e-pharmacies makes it easier for them to circumvent regulations, potentially putting customers at risk, as drug impurities can lead to toxic outcomes.
Undeclared Drug Components
The FDA also issued warnings to major retailers like Amazon and Walmart for distributing non-steroidal anti-inflammatory (NSAID) products containing undeclared drug ingredients, such as diclofenac and dexamethasone. The health implications of combining prescription and non-prescription medications, known as polydrug use, can be significant, especially when all components are not disclosed. Polydrug use can negate the intended effects of medications or shorten the effective dosing period. Furthermore, research has identified adverse effects associated with polydrug use involving NSAIDs, including gastrointestinal bleeding, platelet function impairment, and increased cardiac risks.

While online pharmacies have contributed to reduced costs and improved patient accessibility, it is imperative that enforcement of regulations is prioritized. The responsibility for ensuring the safe and effective delivery of pharmaceutical products through vigilant monitoring and maintenance lies with all stakeholders involved in the online pharmacy landscape.
